This form is eligible for official Shuttle distributors and direct customers only.
Please make sure to have your RMA number provided by Shuttle written clearly on all shipping documents (bill of lading, packlist, delivery note) and on each item that is to be returned.
It is absolutely required to have the invoice from the Shuttle distributor as well as a brief, but significant error report added to the consignment.
These RMA requests fall under our General Service Terms (RMA).
